Première Urgence Internationale (PUI) is a non-governmental, non-profit, non-political and non-religious international aid organization. Our teams are committed to supporting civilians’ victims of marginalization and exclusion, or hit by natural disasters, wars and economic collapses, by answering their fundamental needs. Our aim is to provide emergency relief to uprooted people in order to help them recover their dignity and regain self-sufficiency. The association leads in average 200 projects by year in the following sectors of intervention: food security, health, nutrition, construction and rehabilitation of infrastructures, water, sanitation, hygiene and economic recovery. PUI is providing assistance to around 7 million people in 22 countries – in Africa, Asia, Middle East, Eastern Europe and France.
Première Urgence Internationale has been operating in Libya since 2017, particularly in the regions of Benghazi and, from 2019, in Al Kufra, to strengthen the resilience of the most vulnerable populations affected by ongoing crises, improve their access to essential services. The organization’s intervention in the country is built upon an integrated approach combining the provision of comprehensive primary healthcare services, psychosocial support, protection assistance, WASH activities, and infrastructure and rehabilitation. Première Urgence Internationale develops public infrastructures’ rehabilitation and restoration in areas affected by several years of conflict. Additionally, during the COVID-19 crisis in Libya, the organization adapted its intervention to include Infection Prevention and Control and COVID-19 awareness activities. |
Job Description |
Overall purpose |
Under the direct supervision of the Community Health Officer and under the overall supervision of the Health Activity Manager, the Community Outreach Nurse (CON) is responsible for the screening and referral of beneficiaries at risk of or diagnosed with Non-Communicable Diseases (such as Hypertension and Diabetes) in the areas of intervention. Beneficiaries at risk will be identified through outreach activities in the catchment area of the NCD clinic, and provided with health education and counselling, and referred to the primary health care facility supported by the project. A social and behavourial change approach will be adopted in the implementation of the community health activities. |

Specific objectives and linked activities |
1. Community health interventions: Community Outreach & Referrals
· Under the supervision of the Community Health Officer (CHO), the Community Outreach Nurse (CON) is part of the community outreach team, s/he is in charge of door-to-door NCD preventative and screening activities, providing proper health evaluation and morbidity risk assessment to individuals meeting the NCD risk criteria and at community level. · S/he is responsible to the referral of the detected at-risk beneficiaries to the NCD clinic, to access the full package of NCD care, informing about the services available at PHCC level. S/he monitors the referrals, using tracking documents and coordinate with the health team inside the health facility for the respective case follow-up. · The CON will support training modules for building the capacity of the community health workers – and other assigned staff – in their area of expertise/topic (such as behavioural change for non-communicable diseases, communicable diseases, or any other topic of community Health relevance) · S/he supports the development of educational material (e.g., field assessment/testing) for assigned topics of public health impact. · S/he provides person-centred, evidence-based nursing care related to NCD, following PUI protocols and tools, being responsible for the quality of the screening process and clinical assessment done to the beneficiaries. · S/he ensures the proper filling and documentation of the collected data from the visits, using the tools and forms created for each process. S/he ensures proper archiving of all the documents. · S/he ensures patients data confidentiality all the time, following the medical ethics before, during and after performing the clinical Nursing practices. · S/he contribute to raise the level of health awareness within the targeted community, providing health awareness sessions oriented to social and behavioural changes. · In collaboration with the CHO, s/he develops a map of the catchment area (community map) identifying the most vulnerable households/individuals to prioritize interventions · S/he participates actively in the health awareness events ensuring the health messages delivery in an interactive approach. · S/he participates to workshops on NCD national protocols, or any related building capacity training designed in the project. · S/he ensures, in collaboration with the medical team, that patient care is optimal, and ensures that patients have appropriate management plans; · S/he guarantees the implementation of monitoring and evaluations tools, according to the defined needs of the project/s, and under the supervision of the Community HO and the overall supervision of the Health AM;

2. COORDINATION AND REPORTING
· Collect necessary information from CHWs on identified NCDs and report accordingly. · Make sure data registration is accurate on hard and digital copies. · Keep up to date the first line manager about changes observed in the field to ensure continuous learning process and activities adjustment. · Reports concerns or requests from patients or their families about PUI services; · Attend PUI team meetings when required · Collaborate with other organisations when required;

3. REPRESENTATION OF PUI
ENSURE THE REPRESENTATION OF PUI BEFORE HEALTHCARE PROVIDERS · Represent the organization before local actors involved in the implementation of project’s activities, and ensure good relations are maintained with each of them (consistent with PUI principles of neutrality and independence); · Do not act out of his/her role in representing PUI before external actors, and reports any concerns regarding PUI representation (ie. External actors, journalists, local authorities) with the health AM · Under the supervision of the health AM, in the event of visits from donors to health facilities, participate actively in the preparation and the smooth organization of the visit when requested;

4. OTHER DUTIES
· Be aware of the security rules in the field at any time and provide feedback to the line manager · Follow safety and security rules on-base; · Be aware and apply the Internal Regulations (IR) PUI for the mission;
In General, the Collaborator has to remain flexible and to perform any other duties as required for the good care of patients
